1. The Fallen. Megatron gets heavily damaged but escapes. Optimus Prime also dies, but is later ressurected by the Matrix.


2. I may be wrong but I thought I saw every single robot die and come back to life at least once in that movie. Some of the humans die and come back to life as well.

   There are about 200 resurrections and Jesus isn't even in it!

4. Octomus Prime does, but comes back to life at the end of the movie, and then all the septacons die.
